1. Lead With Value

Tell people upfront what they get for completing the form. "Complete this 2-minute survey and receive a 15% discount code" is infinitely more compelling than "Please fill out our survey."

2. Remove Every Non-Essential Field

Be ruthless. For every field, ask: "Would we make a different decision without this data?" If the answer is no, remove it. You can always collect more data later.
